New telecommunications carriers must register with the Federal Communications Commission (FCC) when they begin to provide service by filing FCC Form 499-A (Annual Telecommunications Reporting Worksheet) with the Universal Service Administrative Company (USAC). Registration includes contributor identification information, regulatory contact information, agent for service of process, FCC Registration Number (FRN), and FCC contact information. New carriers must identify an agent for service of process within 30 days of providing service. Upon registration, all carriers must update their information within one week if their contact in Washington, D.C. (the D.C. Agent) changes. FCC Form 499-A is due in April each year regardless of contact changes.
